The deadline to file a Claim Form is November 30, 2026. To file a claim online, you must use the Class Member ID and Pin provided on the Notice mailed to you.
Class Members can file a Claim for the following benefits:
Expense Reimbursement: Settlement Class Members may submit a claim for reimbursement for certain documented losses, i.e., money spent or fees incurred for documented out-of-pocket losses with supporting documentation related to the Data Incident (up to $4,000) upon presentment of third-party documentation evidencing such losses; OR
Cash Payment: Settlement Class Members may claim a cash payment in the amount of $45.00; AND
Identity Theft Protection: In addition to electing any of the other benefits, all Settlement Class Members may claim three (3) years of identity theft protection, which includes $1,000,000.00 in identity theft protection insurance.
